Defense and Security Collaborations

The background questionnaire collects extensive information about personal history and foreign contacts.  You will complete the Standard Form 86 (SF-86) through the Electronic Questionnaires for Investigation Processing (eQIP) system.   To prepare for eQIP, you may review the SF-86 and refer to the Guide for the SF-86 for full instructions on completing the SF-86.

You will first receive an email from the Savannah River Site Security Office with general instructions for the eQIP.  You will then receive a computer-generated email containing a 14-character Registration Code.  This code will be requested when answering your Golden Questions (Security Questions) on eQIP. At this point, you can access eQIP to establish your Username and Password and complete your SF-86.

Please note, you must complete the initial e-QIP request within 10 days from receipt of notification.  All forms should be uploaded into the e-QIP system unless otherwise directed.
