{"id":145,"date":"2020-11-10T21:59:11","date_gmt":"2020-11-10T21:59:11","guid":{"rendered":"https:\/\/research.uga.edu\/ohsp\/?page_id=145"},"modified":"2026-04-09T12:30:38","modified_gmt":"2026-04-09T12:30:38","slug":"enrollment","status":"publish","type":"page","link":"https:\/\/research.uga.edu\/ohsp\/enrollment\/","title":{"rendered":"Enrollment Process"},"content":{"rendered":"<div class=\"wpb-content-wrapper\">[vc_row][vc_column][vc_column_text css=&#8221;&#8221;]\n<h2>R-OHSP Enrollment Details<\/h2>\n<h3>Can I enroll in the R-OHSP?<\/h3>\n<p>The R-OHSP is open for enrollment to:<\/p>\n<ul>\n<li>Anyone on an Institutional Animal Care and Use Committee (IACUC)-approved Animal Use Protocol (AUP)<\/li>\n<li>Anyone on an Institutional Biosafety Committee (IBC)-approved protocol<\/li>\n<li>All UGA employees that have potential contact with human bloodborne pathogens, as direct result of their research and\/or instruction activity<\/li>\n<li>All animal husbandry employees that work for either PHS Assured units or non-PHS Assured units at UGA (e.g. Animal Health Research Center (AHRC), University Research Animal Resources (URAR), Nonhuman Primate Facility (NHPF), agricultural facilities)<\/li>\n<li>All persons who enter the NHPF but who will have no direct contact with animals<\/li>\n<li>All members of UGA\u2019s IACUC or IBC<\/li>\n<li>All employees of the Office of Research Integrity and Safety who have potential contact with animals used in research and\/or instruction, biohazards and\/or hazardous chemicals<\/li>\n<li>Anyone entering a UGA research animal facility and having no direct animal contact (open to non-contact enrollment only)<\/li>\n<\/ul>\n<h3>How do I enroll in the R-OHSP for the first time?<\/h3>\n<p>Enrollment in the R-OHSP requires submission of a Research Occupational Health Questionnaire (ROHQ).<\/p>\n<p>To access the ROHQ:<\/p>\n<ul>\n<li>Navigate to the&nbsp;<a href=\"https:\/\/pep.uga.edu\/\" target=\"_blank\" rel=\"noopener\">Professional Education Portal (new tab)<\/a>&nbsp;and login with your UGA MYID credentials.<\/li>\n<li>In the upper right search bar, search for \u201c<strong>Research Occupational Health Program.<\/strong>\u201c<\/li>\n<li>Select the&nbsp;<strong>Research Occupational Health Program Information<\/strong>&nbsp;course and choose \u201cLaunch.\u201d\n<ul>\n<li>Choose \u201cMark Complete.\u201d<\/li>\n<li>Complete the acknowledgement process to add the course to your completed transcript.<\/li>\n<\/ul>\n<\/li>\n<li>Within the displayed document, access the&nbsp;<strong>Research Occupational Health Questionnaire<\/strong>&nbsp;hyperlink.<\/li>\n<li>Complete the questionnaire and choose submit.<\/li>\n<li>After the questionnaire has been risk assessed, the participant will receive an email from the R-OHSP nurse with recommended services and instructions on how to schedule an appointment via PEP, if services are needed.<\/li>\n<\/ul>\n<p>You will need to update your Occupational Health Enrollment annually, or more frequently if you have a change in the nature of your work or your personal health status.<\/p>\n<h3>What is the Re-enrollment Process?<\/h3>\n<p>The&nbsp;<strong>Research Occupational Health Program Information<\/strong>&nbsp;course must be on your&nbsp;<strong>completed<\/strong>&nbsp;PEP transcript.&nbsp; If it is still on your&nbsp;<strong>active<\/strong>&nbsp;PEP transcript, you must \u201cMark Complete\u201d and follow the acknowledgement process to move the course to your completed transcript.<\/p>\n<p>Re-enrollment Steps:<\/p>\n<ul>\n<li>Navigate to&nbsp;<a href=\"https:\/\/uga-sbx.kualihub.com\/build\/space\/5e47518b90adda9474c14adb\/my\" target=\"_blank\" rel=\"noopener\">My Documents in your Kuali Workspace (new tab)<\/a><\/li>\n<li>Open your most recent Completed Occ Health Questionnaire under the \u201cSubmitted\u201d tab.<\/li>\n<li>Choose \u201cDuplicate\u201d in the upper right corner.<\/li>\n<li>Update your information and \u201cSubmit\u201d<\/li>\n<li>After the questionnaire has been risk assessed, the participant will receive an email from the R-OHSP nurse with recommended services and instructions on how to schedule an appointment via PEP, if services are needed.<\/li>\n<\/ul>\n[\/vc_column_text][\/vc_column][\/vc_row]\n<\/div>","protected":false},"excerpt":{"rendered":"<p>[vc_row][vc_column][vc_column_text css=&#8221;&#8221;] R-OHSP Enrollment Details Can I enroll in the R-OHSP?
